Atrial tachycardia in a patient with mechanical mitral valve

A young man presents with palpitation and ECG shows narrow QRS tachycardia with the rate of 140 bpm. It is an atrial tachycardia with 2/1 conduction to the ventricle ( red arrows). Adenosine may hep unmask P waves in case of atrial tachycardia.

    QRS also looks like wpw pattern especially in inferior and V4-V6 leads. What do you think about it?

      Good point. It is because flutter waves are distorting the first part of QRS. If you look at V1, you can easily see P waves and there is no "pre-excitation".
